Completes initial comprehensive and ongoing assessment of patient and family to determine best services to meet their needs
Coordination of services with the patient, family, and care giver team

Registered Nurse (RN) license NYS
BSN required
3 + years of clinical experience
Home care experience
Minimum 1 year of supervisory experience
Excellent observation, problem solving and communication skills
