American Hospice and Home Health Services is seeking a Part Time RN to service in Solano County. Salary is negotiable with hourly and per-visit pay rates depending on experience.

The Registered Nurse coordinates and directs the home care patients care based on individual patient needs. The RN independently manages the Home Health/Hospice patient population using advanced assessment, teaching, and decision-making skills. The nurse ensures appropriate referrals to other services, facilitates interdisciplinary conferencing, and completes required documentation. Relevant knowledge and experience are applied to new patient populations.

Responsibilitiesand Duties
- The Home Health Primary Nurse must possess excellent communication skills, both written and verbal.
- Provide comprehensive assessment, planning, implementation, and evaluation for a caseload of home care clients as the primary nurse.
- Maintain a complete record for the care given with the patient, documenting in the clinical record per agency policy and procedure.
- Keep an updated clinical record on each patient, meeting deadlines for documentation, supervision reports, care plan updates, progress toward goals, and verbal orders.
- Accurately complete admission and revisit documentation, communications, billing, employee statistics, and payroll documentation.
- Document patient, family and caregiver teaching.
- Complete initial assessments of patient and family to determine home care needs; provide a complete physical assessment and history of illnesses.
- Regularly re-evaluate patient nursing needs and revise the plan of care as patient status changes.
- Identify discharge planning needs as part of the care plan and implement prior to discharge.
- Act as Case Manager when assigned and coordinate patient care for the assigned caseload.
- Participate in on-call duties as defined by policy.
- Ensure arrangements for equipment and other necessary items and services are available.
- Instruct, supervise, and evaluate home health aide care provided every two weeks.
- Current RN License in the State of California
- Minimum 1 year of recent professional nursing experience, in home health, hospice, or public health
- Current CPR certification
- Licensed driver with an automobile insured as required
