Full Time Physical Therapist Case Manager. In partnership with other clinicians on the case, you will provide skilled services to evaluate and address the needs of individuals in their homes, so they can live safely and productively in their home environment.
Your goals as a Physical Therapist are to restore function to prevent or minimize disability following disease or injury and to help the client reach his/her maximum performance/function within the limits of his/her capabilities. You will evaluate and treat patients using medically prescribed physical therapy programs. You may also supervise Physical Therapy Assistants (PTAs) per agency and regulatory standards. What you’ll do as a Home Health Physical Therapist Case Manager- Perform an evaluation in the home and develop a care plan and work with PTAs to follow the care plan.
- Help patients with mobility, movement, and regaining independence.
- Create detailed treatment and care plans for each patient.
- Communicate and coordinate with patients’ physicians to ensure a high level of care.
- Evaluate and report patients’ progress to the physician (MD).

- Current unrestricted CA Physical Therapist License
- Current BLS/CPR Certification
- Valid CA Driver's License
- Knowledge of local community resources
- Understanding of how illness/injury affects individuals and how culture influences health care
- Knowledge of state and federal homecare regulatory guidelines (preferred)
- Experience in home health care (highly preferred)
- Experience with high acuity patients (a plus)
- Experience working with an interdisciplinary team (highly desired)
- Case Management fundamentals (highly desired)
- Self-directed, empathetic, critical thinker with outstanding interpersonal skills
- Strong organizational and leadership abilities

Wage range:
Physical Therapist, Level 1: - plus 10K annual bonus, paid quarterly. Starting pay is commensurate with relevant experience above the minimum requirements. 21st Century offers generous bonus incentive plans, comprehensive health benefits, and 401k (up to 4% match).
As a practice, the majority of PT Case Managers are hired as PTIs. Once it is determined that a PTI can supervise 2 PTAs, take on a larger case load and assume a higher level of decision making and autonomy, the individual will be promoted to a PTII with a minimum salary of 135,000 plus a 20K bonus (Paid Quarterly).
