The Community Support Facilitator (CSF) will work on-site in Vallejo, CA, in a full-time capacity. The CSF will provide direct support to individuals with intellectual disabilities by assisting them with daily living tasks, developing independent living skills, and facilitating active community participation. Responsibilities include helping participants with their personalized goals, conducting regular progress assessments, coordinating with their support network, and promoting overall well-being.
The role requires strong organizational and communication skills to ensure quality care and support for participants.
- Experience in caregiving, disability support, or community services
- Strong communication, interpersonal, and organizational skills
- Ability to work collaboratively with participants and their support network
- Familiarity with progress tracking and goal-setting processes
- Patience, empathy, and a commitment to fostering independence
- Bachelor's degree in Social Work, Psychology, or a related field is preferred
- Valid driver’s license and reliable transportation
- CPR and First Aid certification, or willingness to obtain
- Flexibility to fulfill scheduled on-site duties and support participant needs
