About the Position
Physical Therapist Skilled Nursing/Rehab Vallejo, CA (Repost, Increased Need)
Role overview A Physical Therapist is needed for a full time day shift contract at a skilled nursing/post acute rehab facility in Vallejo, CA, treating adult and geriatric residents. This is a 13 week new opening (repost, increased need) focused on short term rehab and long term care in a SNF setting.

Schedule:
8:00 AM 4:30 PM, 30 minute unpaid meal break
Days:
Sun, Mon, Tue, Wed, Thu, Fri, Sat (actual work days per facility schedule)
Guaranteed hours: 35 hours per week
Gross Blended Pay Rate: $60 - $72

Key responsibilities Perform comprehensive PT evaluations (strength, ROM, balance, gait, transfers, functional mobility) for adult/geriatric residents.
Develop and implement individualized plans of care to improve mobility, independence, and functional safety in the SNF setting.
Provide evidence based interventions including therapeutic exercise, gait and stair training, neuromuscular re education, and balance/coordination activities.
Collaborate with the interdisciplinary team (OT, SLP, nursing, social services, physicians) on care planning and discharge.
Accurately document evaluations, daily treatments, progress notes, and discharge summaries in the EMR.
Educate patients and caregivers on home exercise programs, assistive device use, and fall prevention strategies.
Required qualifications and skills Active California PT license (or eligibility prior to start).
Graduate of an accredited Physical Therapy program (DPT, MSPT, or BSPT).
Prior SNF or post acute rehab experience preferred; 1+ year ideal for a 13 week contract.
Current BLS/CPR certification for healthcare providers
