About the Role:
The Care Manager-Youth Program provides comprehensive case management services to at-risk and unhoused youth in the community. This role supports the McKinney-Vento process, working closely with the McKinney-Vento Liaison to ensure educational stability and access to essential resources. The Care Manager assists youth with education and employment opportunities, connects them to housing resources, and facilitates entry into the Continuum of Care to help secure placement on the housing list.
Additionally, this role supports youth in obtaining vital documents such as birth certificates and Social Security cards while coordinating referrals for mental health, medical care, and other supportive services. Using a trauma-informed, person-centered approach, the Care Manager ensures equitable and individualized services that empower youth to achieve stability and self-sufficiency.

Expected schedule:
- Spring/Fall/Winter:
Monday-Friday 12 am - 7:30 pm; 35 per week (7 hour workday); occasional weekends required - Summer:
Monday-Friday 8:30 am - 6 pm; 35 per week (7 hour workday); occasional weekends required
Vallejo, CA - in office & community; 1 remote day may be granted after 90 day introductory period

- Carry a caseload of 18-22 participants maintaining regular contact (at least monthly, based on acuity level) with program participants.
- Coordinate bi-monthly check-in meetings with the youth participant and monthly collaboration sessions with the youth participant's caregiver/guardian
- Assist eligible youth in navigating the McKinney-Vento process and liaise with McKinney-Vento school district representatives to insure enrollment into the CoC for housing and care management assistance
- Provide job readiness support, including resume building, interview preparation, and job placement assistance
- Connect youth with vocational training, internships, and employment programs
- Serve as an initial entry point into the local Continuum of Care for youth experiencing homelessness
- Assist in gathering necessary documentation for housing applications and facilitate placements when applicable
- Support youth in obtaining vital records such as birth certificates, Social Security cards, and identification
- Provide referrals for mental health, medical care, and substance use treatment as needed
- Collaborate with community organizations to enhance available resources for youth and families
- Conduct regular case sessions to monitor progress and adjust service plans as needed
- Bachelor's degree in Social Work, Psychology, Human Services, or a related field OR an equivalent combination of education and experience
- At least 1 year of experience working with at-risk youth, homeless populations, or related human services
- Knowledge of McKinney-Vento Act and available community resources
- Strong organizational, communication, and problem-solving skills
- Ability to work independently and collaboratively in a fast-paced environment
- Proficient computer skills, including Google Workspace and case management software (EHR platforms, HMIS)
- Advanced typing skills
- Experience working in diverse settings with people across all socio-economic spectrums and a wide variety of personalities
- Access to reliable personal transportation required, including a DMV record that permits the driver to be insured under 4th Second's automobile coverage
- Pass Required Background Checks:
Successfully complete Live Scan and other background checks as a prerequisite for hiring, ensuring compliance with safety and security protocols set forth by 4th Second
- Master's degree in Social Work, Public Administration, or a related field
