Job Description:

Provide therapy services to home health clients, including assessment, treatment, and evaluation within a 60-mile radius around Valley City, ND.
Responsibilities:
Provide physical therapy services to home health clients with various conditions such as surgeries and falls.
Document patient progress using the Home Care/Home Base EMR system.
Work independently and manage a caseload of 5-7 patients per day.
Requirements:
Current ND Physical Therapy License and BLS certification.
Minimum 1 year of experience in physical therapy with previous home health experience.
Valid driver's license and auto insurance.
