Position Summary

Provides support services and interventions face-to-face, within the home and community for individuals (children & adults) experiencing mental health or substance use-related crisis. This individual will need to be prepared to provide immediate, short-term and evidence-based practices to screen, assess, stabilize, and make appropriate referrals, including crisis stabilization units, inpatient hospitalization, and acute detoxification services. You will collaborate with the Community Support Services and other departments at PSS to support transitions of care for consumers.

The Mobile Crisis Team operates 24/7 during the week- days, evenings, and night shifts, and weekend shifts available.

Position Summary

Provides in-person crisis assessment, intervention, and stabilization services for children and adults experiencing mental health or substance use–related emergencies. Services are delivered in a variety of settings, including on site at the Porter-Starke Services Crisis Center and throughout the community, such as hospital emergency departments, correctional facilities, clients’ residences, and other community-based locations. A crisis may be defined as substance intoxication or overdose, serious mental illness or serious emotional disturbances, suicide attempts and ideation, other risk of harm to self, risk of harm to others, disruptive behavior, domestic violence, and other crises as defined by the individual.

Uses a trauma-informed, strength-based approach and de‑escalation techniques to lower risks and resolve the crisis if possible so that a higher level of care is not needed. Prepared to provide immediate, short‑term and evidence‑based practices to screen, assess, stabilize, and make appropriate referrals, including crisis stabilization units, inpatient hospitalization, and acute detoxification services. Completing outreach attempts to provide follow‑up referrals that ensure locating, coordinating, and monitoring necessary behavioral health, physical health, and addictions care and services.

Collaborates with Community Support Services and other departments at PSS to support transitions of care for all consumers.
