We are seeking a Nurse Practitioner to provide comprehensive primary care to individuals and families, with a strong focus on health promotion, disease prevention, and coordinated care. This role works collaboratively with clinical staff and other healthcare professionals in a team-based environment.
Key Responsibilities- Develop and implement effective patient care plans
- Manage patient care to ensure continuity, including follow-ups and coordination of services
- Provide whole-person care, including referrals to specialists and other healthcare providers
- Maintain accurate, concise, and timely documentation of patient care
- Perform and document routine health assessments and preventive screenings
- Educate patients on diagnoses, treatment plans, medications, and preventive care
- Adhere to professional standards, including punctuality, timely communication, infection control practices, and patient confidentiality
- Meet established patient volume expectations for outpatient visits
- Participate in quality assurance and performance improvement initiatives
- Attend team meetings, clinical meetings, and required trainings
- Contribute to the development and maintenance of clinical protocols and guidelines
- Engage in community outreach and education when requested
- Represent the provider team in meetings or community settings as needed
- Support teaching and mentoring of students as assigned
- Maintain required continuing education for certification and licensure
- Perform additional duties as assigned
- Graduate of an accredited Nurse Practitioner program
- Master’s degree in Nursing (MSN or higher)
- Active Registered Nurse (RN) license
- Certified as a Nurse Practitioner by a recognized certifying body
- Prescriptive authority and DEA certification
- Current CPR certification
- 1–3 years of experience as a Nurse Practitioner preferred or significant RN experience in a high-acuity setting (e.g., ER or ICU)
- Strong patient-centered care and customer service skills
- Excellent organizational and time management abilities
- Ability to work effectively in a collaborative, team-based environment
- Strong written and verbal communication skills
- Flexibility and adaptability in a clinical setting
- Bilingual (Spanish) preferred but not required
- Experience with electronic health records (EHR) systems
- Proficiency with Microsoft Office and standard office equipment
- Valid driver’s license and reliable transportation
- Participation in required training and quality improvement initiatives
