We are growing and looking for a compassionate, dependable, and skilled PRN Licensed Practical Nurse (LPN) to join our Home Health team. If you are passionate about caring for others and want to make a meaningful difference in the lives of seniors each day, we would love to meet you.

As a PRN Licensed Practical Nurse (LPN), you’ll play an important role in supporting the health, safety, and independence of our residents within their apartment homes throughout the community.

• Medication management and administration
• Performing treatments and nursing procedures as ordered
• Monitoring resident conditions and completing assessments
• Responding appropriately to emergencies and resident needs
• Communicating changes in condition to the nursing team and physicians
• Assisting with activities of daily living (ADLs) when needed
• Supporting and guiding Home Health Aides and caregivers
• Maintaining accurate clinical documentation and records
• Promoting infection control and safety practices
• Creating a calm, respectful, and positive environment for residents

This role includes standing, walking, assisting residents with mobility and daily activities, and occasional lifting up to 50 pounds. You’ll work in a variety of home-like settings throughout our community while following safety, infection control, and resident care protocols.

• Current and valid Licensed Practical Nurse (LPN) license in the State of Indiana
• Valid Indiana driver’s license and reliable transportation
• High School Diploma or equivalent
• Ability to maintain confidentiality and follow HIPAA guidelines
• Strong communication, organizational, and clinical decision-making skills
• CPR certification (or willingness to obtain — we provide certification if needed!)
• Previous home health experience
• M inimum of two (2) years of nursing experience preferred
