As a private duty nurse on our home health team, you’ll care for medically complex patients, primarily children, in a personalized, low-stress environment.

• Provide skilled nursing care under the direction of an RN
• Administer medications and treatments based on individualized plans
• Educate and support families in-home care techniques
• Observe and report changes in patient condition
• Document visits using our easy-to-use EMR system

A qualified Licensed Practical Nurse (LPN) administers skilled nursing care to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Registered Nurse (RN). Services are furnished in accordance with Agency policies. In this role, you will report to the Director of Nursing (DON).

• Average computer skills with competency in media communications
• Must have the physical abilities to perform the described duties
• Acceptance of vision, philosophy and goals of the Company
• Ability to exercise initiative and independent judgment
• Excellent verbal and written communication

• Graduate of an approved school of practical nursing and currently licensed in the state(s) in which practicing
• One year experience in nursing (preferred)
• Active and valid CPR certification with practicum
• IV certification by state mandate, if required by the state
