Position: Travel Nurse RN - Med Surg - $2,380 per week

GHR Healthcare is seeking a Travel Home Health Registered Nurse (RN) to serve Valparaiso, Indiana and surrounding counties of Northwest Indiana on a 13‑week contract. The position requires 8‑hour shifts, 40 hours per week, and involves comprehensive home‑care nursing responsibilities.

    Valparaiso, IN (46383) – serving Northwest Indiana & surrounding counties
  • Assignment Length: 13 weeks (April 26, 2026 – July 25, 2026)
  • Shift: Monday – Friday, 8:00am – 4:30pm (8‑hour shifts, 40 hrs/week guaranteed 30–40 hrs)
  • On‑Call/Weekend:
    Rotating weekend on‑call
  • Travel:
    Mileage reimbursed between patient visits (valid driver’s license and reliable vehicle required)
  • Dress Code:
    Black scrubs
  • Weekly Estimated Pay: $2,120 – $2,230
Job Requirements / Qualifications
  • Active Registered Nurse (RN) license in Indiana
  • Associate’s degree in Nursing or higher (ADN or BSN preferred)
  • Current Basic Life Support (BLS) certification
  • Minimum 1 year of nursing experience; home health or homecare experience preferred
  • Experience with Homecare Homebase (HCHB) and OASIS training is a plus
  • Strong communication, critical thinking, and professionalism in home health settings
  • Ability to work independently in patient homes and travel within assigned counties
Primary Responsibilities
  • Perform comprehensive home health assessments and ongoing monitoring of patient health status in the home setting
  • Provide skilled nursing visits (wound care, medication management, IV therapy as applicable) and implement physician orders accurately
  • Document care timely and accurately using electronic health record systems (EHR), including HCHB when applicable
  • Coordinate interdisciplinary care with physicians, therapists, case managers, and family members to support patient‑centered plans
  • Participate in weekly team meetings and case conferences to review patient progress and care plans
  • Educate patients and caregivers on disease management, medications, wound care, and home safety
  • Meet productivity expectations (25–30 points per week) while maintaining high‑quality clinical care and compliance with home health regulations
