We are seeking a compassionate, dependable, and skilled Licensed Practical Nurse (LPN) to join our growing Home Health team. In this role, you will provide skilled nursing care to residents receiving home health services while working collaboratively with our interdisciplinary team to promote independence, comfort, and exceptional quality of life. Home health LPNs provide skilled nursing care, administer medications and treatments, monitor changes in condition, and communicate effectively with the supervising nurse and physicians to ensure continuity of care.

Position Summary

The Home Health Licensed Practical Nurse provides direct nursing care and services to residents in accordance with physician orders, individualized care plans, and established nursing standards under the direction of the Registered Nurse and Home Health Administrator.

  • Administer prescribed medications and treatments according to physician orders.
  • Perform nursing assessments and accurately document resident care.
  • Monitor residents for changes in condition and promptly communicate findings to the Registered Nurse and physician as appropriate.
  • Assist with developing, implementing, and following individualized plans of care.
  • Provide wound care and other skilled nursing procedures within the LPN scope of practice.
  • Educate residents and families regarding medications, treatments, and disease management.
  • Collaborate with physicians, Registered Nurses, Home Health Aides, therapists, and other interdisciplinary team members.
  • Maintain accurate electronic documentation and clinical records.
  • Follow all infection prevention, safety, HIPAA, and regulatory requirements.
  • Participate in required in-service education and maintain all required certifications.
Qualifications
Education & Experience
  • Graduate of an accredited Practical Nursing program.
  • Current Indiana Licensed Practical Nurse (LPN) license in good standing.
  • Current BLS CPR certification preferred (FREE certification provided if needed).
  • Valid Indiana driver's license and reliable transportation preferred.
  • Home health experience preferred but not required.
  • Strong communication, assessment, and organizational skills.
  • Ability to work independently while collaborating effectively with the healthcare team.
