VNA Hospice NWI is expanding and seeking compassionate, skilled Registered Nurses to join our interdisciplinary hospice team. As an RN Case Manager, you will coordinate and deliver high-quality hospice care to patients in their homes, ensuring comfort, dignity, and support for both patients and families.
Schedule:
Monday–Friday, 8:00 AM–4:30 PM with on-call requirements;
Service Area includes Porter, LaPorte, Jasper, Lake, Newton & Starke Counties.
