VNA Hospice NWI in Valparaiso, IN seeks a Community Liaison to serve as an ambassador and patient advocate. You will build and sustain relationships with referral sources, hospitals, physician practices, extended care facilities, and community organizations to grow referrals and admissions.
In addition, you will educate partners about hospice services, coordinate outreach with the Marketing team, and maintain accurate records in the Referral Connect CRM.
