Porter-Starke Services, Inc. seeks a PATH Case Manager to conduct outreach to individuals who are homeless or at imminent risk, and to connect them with housing, health care, Medicaid, SNAP, SOAR, and other resources.
The role involves documenting interventions, coordinating with partners, and assisting clients in accessing entitlements. A Bachelor’s Degree is required, with preference for prior community outreach experience.
