NH- RN – Fountain Park Home Health RN

Full time RN in the Van Wert and Putnam county regions. Minimal call time (every 8th weekend and occasionally on weeknights).

• Implementing a resident care plan
• Performing direct nursing services such as administering treatments and assisting in assessments and evaluations
• Assisting with diagnostic and therapeutic procedures
• Consulting with and validating data
• Recording pertinent information in the patient record
• Identifying and evaluating the resident’s response to health problems and treatments
• Communicating residents care needs to others involved in the care
• Assuming responsibility for professional growth and development

• At least 2 year’s experience as RN required
• Home Health experience preferred, but not required
