This role offers a base pay of $125.00/hr your actual pay will be based on your skills and experience.

As part of CVS Health, this role lets you engage with patients in their homes to bridge gaps in care.

The visit lasts about 45 minutes and includes a medication and medical history review, a physical evaluation, and additional diagnostic tests if ordered by the patient’s health plan.

You’ll use an iPad and an intuitive clinical workflow. Prescription, lab orders, and treatment changes are not required.

• Visit millions of people nationwide to connect them to the right care for their unique needs
• Work with Signify Health as a clinician colleague, fulfilling the specified role requirements
• Be flexible to travel locally and within licensed states, as needed
• Conduct virtual visits as needed
• Provide all Signify Health services, including Diagnostic & Preventive Services (e.g., spirometry, DEE, bone density screening)

The primary workplace is the private residence of members. Work safely in homes that may contain pets, secondhand smoke, and other potential allergens.

• Able to cover Portland and surrounding area

Position eligible for additional reimbursement per completed Diagnostic and Preventive Services.
