Position Overview

Healthcare Executive Placements, Inc. (HCEPInc) is seeking a Speech Therapist, Home Health for a W2 Contract position based in Santa Clara, California.

Schedule

Full time, 5 8's Days, M - F, 8:30am-5:00pm

Assignment Duration

13 Weeks with high likelihood of extending

Hourly Rate

This position offers an hourly rate between $52.00 to $57.00 which equates to $2,080 to $2,280 per week (based on a 40-hour workweek.

Summary

The Home Health Speech Therapist (SLP) provides diagnostic, therapeutic, and consultative services to patients in their homes. This role is responsible for evaluating communication and swallowing disorders, developing individualized treatment plans, and delivering evidence-based interventions to improve functional communication, swallowing safety, and quality of life. The Home Health SLP collaborates with interdisciplinary healthcare teams and utilizes electronic charting systems to ensure accurate documentation and continuity of care.

Essential

Functions
  • Conducts comprehensive speech, language, cognitive-communication, and swallowing evaluations in the home health setting.
  • Develops and implements individualized treatment plans to meet patient-specific needs and goals.
  • Provides therapeutic interventions, including strategies for communication, speech, language, voice, and swallowing disorders.
  • Utilizes vital signs equipment and monitors patient status to ensure safe therapy delivery.
  • Documents patient assessments, treatments, and progress in EPIC and Remote Client charting systems in compliance with professional and organizational standards.
  • Educates patients and caregivers on strategies for communication support, swallowing safety, and home exercise programs.
  • Collaborates with physicians, nurses, occupational therapists, physical therapists, and case managers to ensure coordinated, patient-centered care.
  • Monitors patient progress and modifies treatment plans as appropriate to achieve goals.
  • Maintains compliance with safety standards, including fit testing requirements for protective equipment.
  • Participates in quality improvement activities, continuing education, and professional development initiatives.
Qualifications
  • Current American Heart Association (AHA) Basic Life Support (BLS) certification required.
  • Must be fit tested for use of required protective equipment.
  • Active driver's license (DL) and reliable transportation required for home visits.
  • Experience with vital signs monitoring, EPIC, and Remote Client charting systems preferred (training provided if needed).
  • Minimum 1 year of experience in home health, inpatient, or outpatient settings preferred.
  • Strong interpersonal, communication, and patient education skills.
  • Ability to manage an independent caseload and adapt to variable home environments.
