Home Care Aide

We are looking for a Home Care Aide to join our team. You will directly work with patients, following a one‑on‑one care plan in their homes. You will provide personalized, attentive care while building genuine relationships, helping create a healthy environment that meets each patient's needs. Prior Home Health or Hospice experience is a plus.

• Assist patients with daily activities such as moving in and out of beds, baths, wheelchairs, or automobiles
• Care for patients by changing bed linens, doing laundry, cleaning the home, or assisting with personal care
• Maintain records of patient care, condition, progress, and problems to report and discuss observations with a supervisor
• Build relationships with patients by conversing or reading aloud to help keep them mentally healthy and alert
• Give medication reminders
• Engage patients in exercises or other activities

• High School Diploma or GED
• CHHA preferred
• One year prior home health aide experience a plus
• Driver’s license required
• Reliable transportation

Compensation: $13 – $15 per hour
