Home Health Physical Therapy Assistant (PTA)

Eden Health -  - Responsibilities:
Implement treatment plans ordered by the physician as indicated by the plan of care to all patients on caseload;
Review caseload with supervising physical therapist on a regular basis as established by best practice and/or state guidelines;
Educates patients, families/caregivers, and other staff in physical therapy modalities;
Delivers physical therapy modalities appropriate to the patient’s clinical status and needs; in accordance with standard of care;
Completes accurate visit notes via electronic health record on every patient in a timely fashion
