Summary:

The PACT Therapist / Mental Health Professional (MHP) is a core member of a multidisciplinary treatment team that provides intensive, integrated services to adults with serious and persistent mental illness (SPMI) who experience complex barriers to functioning independently in the community. This position delivers a range of clinical services—assessment, therapy, crisis intervention, and case management—both in office settings and in the community, including client homes, shelters, and hospitals.

The goal is to support client recovery, community integration, and psychiatric stability by reducing hospitalization and incarceration and enhancing quality of life.

Clinical & Therapeutic Services

  • Provide integrated mental health and substance use assessments using DSM-5 diagnostic clarity and service planning.
  • Provide trauma-informed, evidence-based treatment for individuals with or without co-occurring mental health and substance use disorders, including but not limited to CBT, DBT, MI, and harm reduction strategies.
  • Conduct comprehensive mental health assessments, including biopsychosocial evaluations and risk assessments.
  • Deliver structured individual and group psychotherapy for recovery support, skill-building, relapse prevention, and psychiatric rehabilitation.
  • Collaborate in the development and revision of individualized treatment plans, crisis plans, and recovery goals.
  • Provide consultation and psychoeducation to other team members, supporting integrated interventions aligned with each client’s readiness for change.
  • Collaborate with the PACT prescriber and team to coordinate medication-assisted treatment (MAT) and monitor progress toward wellness and recovery.
  • Provide ongoing crisis assessment, risk evaluation, and urgent intervention as needed, in collaboration with the team.
  • Provide psychoeducation on mental health and co-occurring disorders, medications, relapse cycles, wellness strategies, and coping skills.
  • Support harm reduction, housing stability, medication adherence, and community integration through are coordination and case management.
  • Promote client autonomy, dignity, and culturally responsive care in all interactions
  • Collaborate with team members to ensure cohesive delivery of mental health services in the community and across levels of care.
  • Assist with discharge planning and coordination of referrals to inpatient detox, outpatient treatment, housing, or residential supports as appropriate.
  • Advocate for clients across systems (legal, medical, housing, substance use treatment) to reduce barriers and support whole-person recovery.
