Summary

The Advanced Practice Registered Nurse (APRN) Certified Nurse Practitioner (NP) provides a full range of services emphasizing quality - timeliness - and the advancement of the Veterans Affairs mission. The NP provides advanced clinical practice - consultation - and management - and practice to the full extent of their education - training - and certification. Core functions are outlined in the clinical privileges - The NP has prescriptive authority for patients or program as indicated by their position.

Duties
  • Major duties/responsibilities include - but are not limited to the following:
    Independently manage a panel of patients with acute and chronic cardiovascular health conditions with an emphasis on health promotion and disease prevention using evidence-based practice;
    Develop and implement an individualized plan to achieve optimal population health and disease management;
    Demonstrate leadership and works collaboratively with interdisciplinary teams within the Cardiology Section - the Department of Medicine and the Medical Center;
    Order and interpret labs, tests, and diagnostic studies;
    Prescribe pharmacologic and non-pharmacologic interventions;
    Initiate referrals and consults to specialty services within the facility and for Advanced Cardiovascular Care in the community;
    Provide cardiovascular education to patients and their families with a focus on acute and chronic heart failure;
    Assess and adjust the plan of care as needed to achieve optimal patient outcomes;
    Independently practice as a NP providing a full range of care utilizing treatment guidelines in the delivery of high-quality care;
    Collaborate with patients, families, and health care practitioners to develop, implement treatment plans, and evaluate health outcomes;
    Demonstrate critical thinking in assessing and managing complex clinical situations;
    Lead the delivery of care to assure continuity of care and peer accountability for practice - improving outcomes of panel/program
  • Lead performance improvement initiatives that improve care or processes. Engage in evidence-based practice and the application of clinical practice guidelines. Implement quality improvement initiatives that lead to outcomes at the panel/program level.
  • Demonstrate positive effective communication skills and professionalism that promote cooperation and teamwork with internal and external customers. Work effectively as a leader in planning and facilitating systematic change. Collaborate with stakeholders to improve patient care, staff development, and performance measures. Participate as a member or leader in formal/informal work groups or interdisciplinary teams. Lead within a High Reliability Organization (HRO) framework. Acquire and disseminate knowledge that impacts patient outcomes.

    Educate and/or mentor colleagues and/or students. Assume responsibility and accountability for evaluating progress toward professional career goals. Exemplify the ICARE core values of Integrity, Commitment, Advocacy, Respect, and Excellence.
