** Description*
* Peace Health is seeking an experienced Nurse Practitioner to join a vibrant Inpatient Palliative Care team in Vancouver, Washington.

The ideal candidate will have one or more years post graduate experience in Inpatient Palliative Care. Candidates with a Palliative Care Certification are highly desired.

** Highlights about the practice:*
* + Join a collegial team of two Nurse Practitioners and a Physician Medical Director, seeing ~4-5 patients daily, with support from RNs and MSWs.

+ 100% inpatient and ability to grow the service line in our community.

+ Southwest Medical Center has on-site complex case management teams, social workers, chaplains, and partnerships with the Ray Hickey Hospice House.

+ Full-time schedule; no call or weekend requirements.

+ Epic EMR.

** Job Summary*
* Within scope of practice and in alignment with both hospital staff bylaws and PHMGs physician supervision expectations, responsible for advanced level of nursing evaluation, clinical assessment, prescribing and monitoring medications and follow-up care for patient. Responsible for patient and family teaching.

** Essential Functions*
* + Effectively identifies, evaluates, and addresses disease prevention and health promotion issues of the population in the practice while administering quality patient care.

+ Works in an independent and interdependent relationship with members of the medical staff, which allows for consultation, collaboration, or referral.

+ Gives directions to nursing and other staff.

+ Prescribes medications and treatments and ongoing medication management. Orders lab and diagnostic tests when appropriate and interprets test results.

+ Educates patients and/or families about preventive care, medical issues, and use of prescribed medical treatments and/or medications.
