Position: RN Hospice House- Ray Hickey Hospice House
** Description*
* ** Peace Health is seeking a RN Hospice House
- Ray Hickey Hospice House for a Part Time, 0.70 FTE, Evening position. The salary range for this job opening at Peace Health is $50.14 - $78.34.
** The hiring rate is dependent upon several factors, including but not limited to education, training, work experience, terms of any applicable collective bargaining agreement, seniority, etc.

** Hiring Bonus may be available!*
* This position is responsible for providing direct skilled nursing care and coordination/management of patients admitted to hospice house inpatient services using considerable judgment and skill in making patient care decisions. Utilizes the professional healthcare process of assessment, problem identification/planning/intervention and clinical/goal outcome evaluation.

** Essential Functions*
* + Perform initial evaluation of patient and apply admission criteria to determine if patient is safe candidate for home health/hospice services.

+ Provide skilled nursing care to patients and skilled education/instruction to patients and caregivers according to the hospice plan of treatment.

+ Performs physical assessments, develops, coordinate, modifies and manages written individualized plan of care, as ordered by the primary physician, to meet patient's long and short-term medical goals and appropriate/timely hospice outcomes.

+ Develops and clinically manages the Home Health Aide's care plan according to the patient's evolving personal are needs.

+ Coordinates care, collaborates and communicates with other members of the interdisciplinary team to assure appropriate and smooth flow of care/services through the care continuum.

+ Performs discharge planning appropriately towards patient specific goals/outcomes and evaluate interventions and outcomes of patient care by reviewing long and short-term goals established in the plan of care.

+ Provide accurate, complete and timely clinical documentation in compliance with regulatory, quality and Agency guidelines.

+ Assist with the plan and implementation of family Bereavement services for the following patient death.

+ Performs other duties as assigned.

* + Accredited School of Nursing

Required:

Nursing  and

+ Bachelor's Degree Preferred:
Bachelor of Science in Nursing

** Experience*
* + Minimum of 1 year

Required:

home health or hospice/end of life nursing experience  and

+ Preferred:
Hospice experience and

+

Required:

Demonstrated knowledge of and ability to apply age specific principles of growth and development and life stages to meet each patient's needs

** Credentials*

Required:

Upon Hire Registered Nurse in state of practice and

+

Required:

Upon Hire Basic Life Support  and

+

Required:

Upon Hire Food Handlers Card - Washington

** Skills*
* + Basic computer skills including MS Office applications (Required)

+ Well organized, flexible and able to function independently (Required)

+ Cooperative working skills, leadership skills, and interpersonal skills and ability to work effectively with a wide variety of patients, families and staff (Required)

** Working Conditions*
* Lifting

+ Patient handling no greater than 35 lbs. without the use of assistive equipment and/or devices (NIOSH).

+ Fine motor skills to be able to grasp and control medical equipment and perform precise procedures.

+ Push/Pull:
Frequently up to 45 lbs. force (i.e., WOW, medical carts).

+ Perform hands on CPR at least 20 mins using 100-125 lbs. of force (National Assoc. of EMS Physicians and AHA) Frequency should be seldom or occasionally.

+ Ability to move around area with frequent sitting.

+ Bending/ stooping/ squatting/ reaching/ kneeling frequently.

+ Lifting (non-patient) up to 20 lbs. occasionally and 5 lbs. frequently.

Environmental Conditions

+ Exposure to biohazard, body fluids and airborne particles.

+ Must be able to complete tasks in a noisy environment.

Mental/Visual

+ Vision and hearing required within normal limits (glasses, contacts, hearing aids permitted).

+ Ability to communicate and exchange accurate information.
