Job Description

Peace Health is seeking a RN Home Health for a Part Time, 0.80 FTE, Day position. The salary range for this job opening at Peace Health is US $52.95 – $80.09. The hiring rate is dependent upon several factors, including education, training, work experience, collective bargaining terms, and seniority. A hiring bonus may be available.

• Perform initial evaluation of patient and home and apply admission criteria to determine if the patient is a safe candidate for home health/hospice services.
• Provide skilled nursing care and education to patients and caregivers according to the home health/hospice plan of treatment.
• Conduct physical assessments and develop, coordinate, modify, and manage an individualized written plan of care, as ordered by the primary physician, to meet the patient’s long‑ and short‑term medical goals and appropriate home health outcomes.
• Manage the Home Health Aide’s care plan according to the patient’s evolving personal and are‑needs.
• Coordinate care, collaborate, and communicate with other members of the interdisciplinary team to assure smooth flow of care/services through the continuum.
• Plan discharge appropriately toward patient‑specific goals and evaluate interventions and outcomes by reviewing the plan of care goals.
• Maintain accurate, complete, and timely clinical documentation in compliance with regulatory, quality, and agency guidelines.
• Assist with the plan and implementation of family bereavement services for patient deaths.
• Perform other duties as assigned.

• Accredited School of Nursing required:
  Nursing
• Bachelor's Degree preferred:
  Bachelor of Science in Nursing

• Minimum of 2 years acute clinical or home health/hospice nursing experience required.
• Minimum of 1 year home health experience preferred, or will orient to competency.
• Demonstrated knowledge and ability to apply age‑specific principles of growth and development to meet patient needs.

• Registered Nurse in the state of practice required.
• Driver’s license and proof of auto insurance required.
• Basic Life Support upon hire required.

• Basic computer skills including MS Office applications.
• Well organized, flexible, and able to function independently.
• Cooperative working skills, leadership skills, interpersonal skills, and ability to work effectively with patients, families, and staff.

Accept valid driver’s license in US or Canada with proof of auto insurance.

• Patient handling no greater than 35 lbs. without assistive equipment or devices (NIOSH).
• Fine motor skills to grasp and control medical equipment and perform precise procedures.
• Push/Pull:
  Frequently up to 45 lbs. force (medical carts).
• Hands‑on CPR at least 20 mins using 100‑125 lbs. of force; frequency seldom or occasionally.
• Ability to move around the area with frequent sitting.
• Bending, stooping, squatting, reaching, kneeling frequently.
• Lifting non‑patient items up to 20 lbs. occasionally and 5 lbs. frequently.

• Exposure to biohazard, body fluids, and airborne particles.
• Must perform tasks in a noisy environment.

• Vision and hearing within normal limits (glasses, contacts, hearing aids permitted).
• Ability to communicate and exchange accurate information.
