Job ScopeA telephone triage nurse assesses patient needs over the telephone, collaborates with a physician or qualified staff member to meet those needs, and documents all elements of care in the patient’s medical record. The telephone triage nurse will accurately identify high‑risk patients, direct care to appropriate resources, and under general supervision provide professional nursing care following national and organizational standards and guidelines.
Responsibilities- Address patient calls, portal messages, and emails in a timely manner concerning a wide range of symptom/side effect related topics.
- Ask appropriate, open‑ended questions and utilize computerized algorithms/pathways to assist in the assessment of patients’ symptom and side‑effect management inquiries and documentation of such interventions.
- Assess urgency of patient symptoms/side effects and address them appropriately, aggressively managing them to prevent unnecessary emergency department and hospital utilization.
- Document all conversations with patients to maintain a comprehensive medical record.
- Provide clear instruction and education to patients via telephone.
- Evaluate and determine if emergency care is needed, or if the patient can be seen in the outpatient clinic.
- Collaborate with physicians via electronic communication, face‑to‑face, or through telephone to discuss patient care needs that cannot be independently and appropriately addressed using standing orders or practice protocols.
- Set up same‑day and/or future provider/infusion room appointments to address symptom management concerns.
- Participate in cross‑training programs to develop skills across multiple areas of the practice and provide support where needed.
- Perform additional duties as assigned.
- Graduate from an accredited professional nursing education program; BSN preferred.
- Dual licensure in Oregon (OR) and Washington (WA) must be obtained within six months of employment start date.
- Current CPR certification required.
- Current or pending Oncology or applicable specialty certification preferred.
- Minimum of 0‑3 years of nursing experience.
- Oncology or applicable specialty experience preferred.
- Experience and knowledge of the purpose and use of medications in the treatment of cancer preferred.
