Peace Health is seeking a RN Hospice Adm/Benefits Coordinator
- Hospice for a Per Diem/Relief, 0.00 FTE, Day position. The salary range for this job opening at Peace Health is $54.65 – $82.66. The hiring rate is dependent upon several factors, including but not limited to education, training, work experience, terms of any applicable collective bargaining agreement, seniority, etc.

Receives referrals and coordinates history and physical by completing intake information sheet, securing accurate demographics and directions; reviews information for appropriateness of referral and confirms insurance coverage; coordinates admission times; the core values are integrated comprehensively into the work environment; maintains current competency requirements, both initial and targeted

• Coordinates home services admission activities by: assisting in the gathering of appropriate authorization and medical criteria to meet CMS guidelines for Home Health and Hospice services; demonstrating clinical knowledge of appropriate cause for referral and criteria for admission by evaluating referrals for suitability/eligibility and discussing with physician; identifying patient's physical, psychosocial and cultural needs in conjunction with referring source; providing resources appropriate to patient's age;
  
  setting up ancillary services, supplies and equipment needed to maintain patient's safety at home.
• Provides care planning/coordination to include: coordinating and confirming insurance coverage; effectively planning with appropriate members of the care team; recommending alternative plan of care to physician and discharge planner if appropriate; maintaining communication with patient care team to facilitate problem-solving about patient issues.
• Performs effective communication and documentation by: communicating and documenting in accordance with standards; utilizing SBAR communication format; keeping abreast of and complying with departmental and organizational changes and events through reading appropriate communications and attending unit and assigned meetings.
• Provides education if appropriate to new employees, volunteers, physicians, families, patients and nurses regarding Home health and Hospice services utilizing available education resources.
• Performs general duties to include: supporting cost containment and financial viability of the department by meeting productivity standards and by appropriate utilization of supplies and resources; maintaining a working knowledge of infection control, risk management and emergency preparedness plans; actively participating in performance improvement initiatives; demonstrating knowledge of hospital, interdepartmental and unit goals, standards and operations.

Education

• Accredited School of Nursing Required
• Bachelor's Degree Preferred:
  Bachelor of Science in Nursing

Experience

• Minimum of 1 year
  
  Required:
  
  hospice or home health experience (or may have equivalent combination of education and/or experience)

Credentials

• Required:
  
  Registered Nurse in state of practice
• Required:
  
  Upon Hire Basic Life Support

Working Conditions

• Lifting Patient handling no greater than 35 lbs. without the use of assistive equipment and/or devices (NIOSH).
• Fine motor skills to be able to grasp and control medical equipment and perform precise procedures.
• Push/Pull:
  Frequently up to 45 lbs. force (i.e., WOW, medical carts).
• Perform hands on CPR at least 20 mins using 100-125 lbs. of force (National Assoc. of EMS Physicians and AHA) Frequency should be seldom or occasionally.
• Ability to move around area with frequent sitting.
• Bending/stoop/squat/reach/kneel frequently.
• Lifting (non-patient) up to 20 lbs. occasionally and 5 lbs. frequently.

Environmental Conditions

• Exposure to biohazard, body fluids and airborne particles.
• Must be able to complete tasks in a noisy environment.

Mental/Visual

• Vision and hearing required within normal limits (glasses, contacts, hearing aids permitted).
• Ability to communicate and exchange accurate information.
