Long Term Acute Care Nurse; LTAC RN

Deliver advanced nursing care for medically complex patients.

We're seeking experienced and compassionate LTAC Registered Nurses to care for patients requiring extended hospital-level treatment. If you're confident managing high-acuity, chronically ill patients and value continuity of care, this role offers meaningful impact every shift.

• Per Diem
• 12-Hour Shifts
  • 7:00 AM – 7:00 PM (Days)
  • 7:00 PM – 7:00 AM (Nights)

• Provide comprehensive nursing care to patients with complex, long-term medical needs
• Perform detailed patient assessments and continuous monitoring
• Manage ventilators, tracheostomies, feeding tubes, central lines, and complex wounds
• Administer medications, IV therapies, titratable drips, and blood products per provider orders
• Monitor and respond to changes in patient condition promptly
• Collaborate with physicians, respiratory therapists, case management, and interdisciplinary teams
• Support patient stabilization, recovery, or transition to the next level of care
• Educate patients and families on disease processes and long-term treatment plans
• Maintain strict adherence to infection control and patient safety protocols
• Document assessments, interventions, and outcomes accurately in the medical record

• Active RN license in the state of practice
• Minimum of 1 year of recent LTAC, ICU, Step-Down, or acute care hospital RN experience required
• BLS certification required (ACLS preferred)
• Strong critical-thinking and clinical decision-making skills
• Experience caring for ventilator-dependent or medically complex patients
• Ability to manage high-acuity patients in a fast-paced environment
• Excellent communication and interdisciplinary teamwork skills
