Nurse Practitioner​/Physician Assistant

? Vitora Healthcare is a physician-led medical group seeking a motivated Nurse Practitioner / Physician Assistant to provide on-site care in skilled nursing and long-term care facilities in the Vancouver, Washington area.

Provide on-site medical care to skilled nursing and long-term care patients, including management of acute and chronic conditions, post-hospital care, and coordination with nursing, therapy, specialists, and facility leadership.

• Washington NP/PA license, or eligibility to obtain one
• National certification and DEA eligibility
• Strong clinical judgment and communication skills
• Efficient, independent, and highly motivated

SNF experience is helpful but not required.
