About

The Role

As a Family Peer in the Wise Program, you’ll be an essential part of the care team, using your lived experience as a parent or caregiver to support families navigating behavioral health and other systems of care. You’ll provide encouragement, mentorship, advocacy, and practical support while helping families feel heard, understood, and empowered. By building trust and connection, you’ll help families navigate services, strengthen support systems, and work alongside youth and care teams to support meaningful outcomes.

What

You’ll Do
  • Program Development: Collaborate with stakeholders to identify program needs, goals, and objectives, and develop engaging educational materials, resources, and curriculum for peer-focused programming.
  • Peer Support Services: Provide peer support services across Three Rivers Therapy programs, locations, and shifts. Support daily program operations, including set-up, food and coffee preparation, and clean-up. Represent Three Rivers Therapy and its programs professionally when interacting with clients, community partners, agencies, and visitors.
  • Education & Facilitation: Lead educational sessions, workshops, and presentations using engaging and interactive approaches. Adapt curriculum and content to meet the needs of participants and ensure information is relevant and accessible.
  • Community Engagement: Build and maintain positive relationships with clients, community members, organizations, and service providers. Partner with community leaders and organizations to strengthen connections and expand opportunities for the people we serve.
  • Documentation & Reporting: Maintain accurate and timely client records, progress notes, case files, and program documentation. Track program activities and participant progress and provide updates and reports to the manager as needed.
  • Advocacy & Empowerment: Advocate for clients’ needs and rights and help connect them with community resources, activities, and support networks that promote social connection, independence, and a sense of belonging.
  • Crisis Support: Provide support, guidance, and appropriate intervention to clients experiencing difficult or crisis situations, following established protocols.
  • Transportation & Community-Based Services: Transport clients to appointments and community resources as needed. Provide services in a variety of community settings and travel throughout the service area to meet clients where they are.
  • Perform other duties as assigned.
