The ideal candidate will have experience in home health and be comfortable working independently while coordinating care with an interdisciplinary team. • Perform comprehensive evaluations and OASIS assessments
• Develop and implement individualized care plans
• Treat patients with speech, language, cognitive, and dysphagia disorders
• Accurately document visits in accordance with Medicare guidelines
• Educate patients and caregivers on exercises and compensatory strategies
• Communicate effectively with physicians and care team members

• Master’s degree in Speech Language Pathology
• Active Florida SLP license
• Home health experience preferred
• Knowledge of OASIS documentation is a plus
• Possesses and maintains current CPR certification
• Valid driver’s license and reliable transportation
