Job Summary
We are seeking a compassionate Medical Social Worker to support home health patients and their families by addressing psychosocial, emotional, and resource needs.
Responsibilities- Assess patient psychosocial and environmental needs
- Assist with discharge planning and long-term care arrangements
- Provide counseling and emotional support to patients and families
- Connect patients with community resources and financial assistance programs
- Complete required documentation per Medicare/home health standards
- Participate in care coordination with interdisciplinary team
- Master’s degree in Social Work (MSW)
- Active Florida license (LMSW/LCSW preferred)
- Home health or healthcare experience preferred
- Strong knowledge of community resources
- Excellent communication and organizational skills
- Flexible schedule
- Mileage reimbursement
This role requires a Level 2 background screening in compliance with Florida AHCA regulations prior to employment.
