Nurse Practitioner​/Physician Assistant - Primary Care

Position Overview

• Provide on‑site, team‑oriented primary care to patients within Assisted Living and Memory Care communities.
• Full‑time providers care Monday‑Thursday;
  Friday can be a clinical or administrative day.
• Manage a panel of 200 patients (approximately 12–16 patients per day with an average of 14).
• Design and manage your own schedule.
• Develop and implement personalized care plans based on patient needs and preferences.
• Receive clinical support from Clinical Assistants and Care Coordination staff (LPNs, RNs, Social Workers).
• Access diagnostics such as labs and X‑rays as needed.
• Collaborate with community partners in home care, hospice, and other resources as appropriate.

• Graduate of an accredited Nurse Practitioner or Physician Assistant education program.
• Board certified or board‑prepared through AANP, ANCC, or NCCPA.
• Licensed or able to obtain licensure through the Board of Nursing or Board of Medical Practice.
• Ability to obtain and/or maintain DEA certification for the state of practice.
• Strong verbal communication skills.
• Excellent interpersonal and customer‑service skills.
• Valid, unrestricted driver’s license and minimum state‑required liability auto insurance; local travel is required.

• 2+ years of clinical experience as a Nurse Practitioner or Physician Assistant.

• Compensation Base + Quality/Productivity: $125,000 – $170,000.
