ScheduleMonday - Friday 8am to 5pm
Under the direction of RN case manager, provides clinical, physical, and emotional support to patients as directed in plan of care, reporting significant changes in patient condition as needed. Provides support to family/caregiver reporting psychosocial/spiritual needs to team.
Primary Tasks, Duties, And ResponsibilitiesUnder the direction of the RN case manager, provides continuous bedside clinical observations, prescribed care, physical, and emotional support to patients as directed in plan of care, reporting significant changes in patient condition and family needs as needed.
- In collaboration with the RN, accepts and processes physician orders to maximize patient comfort.
- The LPN utilizes monitoring, intervention, and teaching skills to maximize patient comfort and enhance the quality of life for the Patient/Family unit.
- Applicants must be a graduate of an approved nursing program and own a current Florida LPN license.
- Minimum one year experience in an acute care setting, providing direct patient care; previous experience as a hospice LPN preferred.
- Completion of an approved 30 hour IV certification course required within 6 months of hire.
