$32.00 - $40.00 hourly, depending on experience.
LocationVentura, Ojai, Fillmore, Oxnard, Camarillo (surrounding areas)
Responsibilities- Implement current nursing practice following a comprehensive assessment and plan of care.
- Provide accurate and timely documentation of patient services to reflect the plan of care.
- Assess and provide patient and family/caregiver education and information related to diagnosis and self‑care plans.
- Participate in coordination of hospice services, reporting identified needs to the interdisciplinary group (hospice aide, OT, PT, SW, ST, Dietician, Clinical Supervisor).
- Use and prepare equipment and supplies effectively, adhering to aseptic technique.
- Provide appropriate pain/symptom management and evaluate patients’ response to treatments and medications.
- Current state license as an LVN/LPN.
- At least one (1) year of experience working as an LVN/LPN.
- Current Basic Life Support (BLS) and hands‑on CPR certification.
- Valid driver’s license and reliable, insured transportation.
