We are looking for a per diem Nurse Practitioner for our Hospice Department in Ventura.
We have about 15–20 face‑to‑face visits per month: $150 per visit plus $11 for mileage.

JOB SUMMARYThe Hospice/PC Nurse Practitioner (NP) is a master’s prepared advance practice registered nurse licensed in the state of California. The NP utilizes assessment and health care management skills in conjunction with the AHH Standardized Procedures and the collegial physician‑nurse practitioner relationship to meet the health care needs of seriously or terminally ill patients. The NP provides expertise in hospice and palliative care as a nursing team leader and member of the interdisciplinary team, and is responsible for providing guidance and leadership to the team delivering palliative care.
DUTIESAND RESPONSIBILITIES
- Under AHH Standardized Procedures for Nurse Practitioners, provide medical care to seriously or terminally ill patients in conformance with the patient’s IDG plan of care and collaboration with the attending MD and Medical Director:
- Perform history and physical exams
- Diagnose and treat medical problems
- Order and interpret diagnostic and laboratory tests, as appropriate
- Educate and counsel individuals, families and groups
- Facilitate appropriate health maintenance activities (limited immunizations)
- Provide the skilled nursing services necessary to adhere to the patient’s plan of care developed by the IDG including the patient’s physician and family
- Ensure adherence to the plan of care for every patient/family assigned to the team
- Perform assessment and identify the patient’s and family’s physical, psychosocial, environmental, safety and developmental needs and individualize the patient’s plan of care based on identified needs
- Assess all aspects of the patient’s pain and develop an individualized pain management plan
- Anticipate, prevent and treat undesirable symptoms, secondary symptoms and other comfort needs
- Support, instruct, and educate the patient, family, caregivers and facility staff as necessary to meet the patient’s needs
- Document problems, nursing assessments, appropriate goals, care provided, care interventions and patient and family outcomes achieved from intervention and care provided
- Maintain the dignity and support the unique beliefs of the dying patient and their family
- Provide holistic, family‑centered care across treatment settings to improve quality of life
- Perform duties consistent with the Nursing Practice Act including the Standards of Competent Performance
- Direct the use of medical supplies and equipment to ensure patient safety
- Supervise RN, CHHA and LVN services as required by law, licensure and agency policy
- Appropriately consult with supervising physician or agency medical director on patient care management
- Comply with regulatory agencies governing hospice
- Document care in conformance with agency policy; submit all documentation within appropriate time frames
- Act as a mentor for the organization’s clinical staff and affiliated nursing and medical students
- Participate in provider and administrative on‑call rotations as appropriate
- Participate in interdisciplinary group meetings
- Protect patient’s rights to confidentiality and privacy in all interactions
- Portray a professional appearance and maintain professional conduct at all times
- Facilitate excellence in clinical services and act as a role model for customer service
- Support the mission and goals of the Agency
- Maintain all required licenses and certifications; pursue professional growth and development
- Abide by Agency policies and procedures
