We are accredited and growing quickly in Ventura County, and we're looking for compassionate Nurse Practitioners to join our team.

- Perform face-to-face evaluations and provide palliative care for adult patients with acute and chronic conditions.
- Evaluate patients and provide follow-up care in collaboration with the supervising physician.
- Assess and manage pain and other symptoms to promote comfort and quality of life.
- Communicate compassionately with patients and families regarding prognosis, goals of care, advance care planning, and advance directives.
- Identify social, emotional, and spiritual needs and coordinate appropriate support and referrals.
- Coordinate hospice services across care settings to ensure seamless, high-quality care delivery.
- Educate patients and caregivers on symptom management, care needs, and available resources.
- Complete accurate and timely clinical documentation within 24 hours of each visit.
- Experience: Current RN licensure and current active Nurse Practitioner Certification in the State of California (required). National Provider Identification Number. Furnishing number issued by the Board of Registered Nursing (BRN) (required). DEA number issued by the U.S. Department of Justice Drug Enforcement Administration required to furnish or order Schedule II-V controlled Substances. Certification as an Adult Nurse Practitioner by a nationally accredited Nursing organization is preferred.
- Clinical foundation: Background in oncology, med-surg, ICU/CCU, or skilled nursing is a plus.
- Values: Understanding of hospice philosophy and the needs of the terminally ill.
- Skills: Excellent interpersonal communication; compassionate, patient-centered approach.
- Work style: Self-directed and dependable with minimal supervision.
- Valid driver's license, auto insurance, and reliable transportation
- Willingness to travel to patient residences across Ventura County
