Sanford Health in Vermillion, SD is seeking a qualified Social Worker to join the care coordination team. You will provide social work services, coordinate programs, and assist patients and families with social, emotional, and economic concerns within an interdisciplinary setting.
Responsibilities include crisis intervention, referrals, and advocacy, with licensure requirements aligned to state practice locations. BLS certification may be required within six weeks depending on the role.
