Come join our collaborative team, we pride ourselves on strong commitment to process improvement and evidence-based care. Our APP's work independently, while having side by side access to our physician hospitalists. This position offers a combination of 12 hour and 8 hour shifts with no night shift requirement. Our Hospitalist team has access to surgical and medical specialists including general surgery, urology, cardiology, pulmonology, ENT and endocrinology as well as onsite and telemedicine support for the management of critical care and stroke patients.
The Hospitalist provides care to hospitalized patients in the inpatient units following established standards and practices. Clinical problems treated by the Hospitalist may include, though are not limited to health maintenance, common acute illnesses, management of stable chronic disease, and patient/family education consistent with the community standard for primary care providers.
ESSENTIAL FUNCTIONS:
- Completes timely History and Physical and documentation
- Orders appropriate lab and diagnostic procedures and studies
- Interprets and integrates data to deliver appropriate therapeutic goals
- Utilizes referral services when appropriate
- Provides education to patients on both acute and chronic care of condition.
- Works in an independent and interdependent relationship with members of the medical staff, which allow for consultation, collaboration or referral
