Description:
The SUD Case Manager will work closely with Universal Access and Outpatient Staff to provide service coordination and supportive counseling to ensure seamless access to clinical services available SUD Case Manager will also implement, support, and coordinate additional SUD services including accessing MAT, residential treatment, outpatient treatment, recovery supports, harm reduction services, and recovery housing.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
CLIENT SUPPORT
- In order to enhance emergency services across the state, the SUD Case Manager will provide screening, triage, initial information gathering, referral, and crisis planning services to facilitate a seamless connection to evidence-based treatment, MAT, recovery supports, harm reduction services, and other indicated prevention, intervention and treatment resources.
- Provides case management, service coordination, supportive counseling, and skill building for assigned clients.
- Reduce barriers to treatment resulting in an excellent customer service experience.
- Be knowledgeable about ASAM levels of care and Vermont system of care and be able to connect individuals to the appropriate resources, including individuals with co-occurring mental health and SUD services.
- Utilize available and existing resources, such as VT Helplink.
- Provides on-going supportive and/or case management functions in accordance with the problems, needs, and the strategies identified within the service plan in order to assist the clients achieve the stated goals and objectives.
- Actively co-facilitate and engage in all SUD program such as UCS Intensive Outpatient Program, Chess Health, MAT and other recovery supports, as a means of supporting client’s treatment plan goals as well as communicating with the other team members of the program regarding client progress. Will show competencies in screening, assessment, engagement, treatment planning, collaboration, referral, motivational interviewing, HIPAA and 42 CFR Part 2, and other evidence-based counseling and professional ethics theories, in order to provide effective care.
CLIENT IDENTIFICATION AND OUTREACH
- Provides outreach services to members of the community who are in need of our services. Develops systems to provide services to area clients. Provides protection and advocacy of clients’ rights whenever possible. Provides clients with referral information and assists with transportation as needed.
INFORMATION MANAGEMENT AND RECORD KEEPING:
- Completes various clinical and administrative forms for billing, information, and clinical records. Completes all case management documentation on time and in a professional manner. Attends all staff meetings, agency meetings, in-services, and clinical trainings as instructed by supervisor. Maintains confidentiality at all times.
COMMUNICATION
- Ensures that all persons involved with provision of services and support to the client receives appropriate communication about the care and planning related to the client.
- Maintain communication and collaboration with crisis team, and other key stakeholders.
- Provides ongoing face to face reviews with the client on a regular basis to assess the progress made in reaching service goals so that the service plan can be modified as necessary to ensure that the goals and objectives are being achieved.
- Maintains collaborative relationships and communication with community referring agencies to formulate appropriate collaborative intervention strategies.
ADMINISTRATIVE, AGENCY, AND CLINICAL TEAM MEETINGS
- May support the function of Universal Access and provide back-up, as requested, for the Universal Access Team.
- Participates in interagency planning and service coordination activities as directed to improve and enhance service continuity and effectiveness for clients.
- Participates in regular interdisciplinary staff meetings, and Substance use Program supervision.
- Participates in regular group supervision and individual supervision as a means of enhancing professional growth, reviewing the provision of clinical services, and addressing administrative issues.
