• Provide in-home therapy services to maximize the independence and safety of home care and hospice patients.
• Work independently while collaborating with a multidisciplinary team.

• Valid VT Physical Therapist license

• Valid driver’s license and reliable vehicle.
• Experience in a home care setting preferred.
