Physical Therapist Per Diem

Rutland Center for Living and Rehabilitation, a premier residential, long-term, rehabilitation, and subacute care provider, is seeking a dedicated Physical Therapist Per Diem to join our team. As a key member of our rehabilitation department, you will be responsible for providing high-quality physical therapy services to our patients in a fast-paced and dynamic environment.

Responsibilities:

  • Develop and implement individualized rehabilitation plans that cater to the unique needs of each patient, with a focus on achieving optimal functional mobility and independence.
  • Collaborate with interdisciplinary teams, including physicians, nurses, and occupational therapists, to ensure comprehensive care and effective communication.
  • Conduct thorough evaluations, assessments, and treatment sessions for patients with various diagnoses and conditions, including but not limited to orthopedic, neurological, and cardiovascular impairments.
  • Provide Per Diem coverage for our rehabilitation department, ensuring seamless continuity of care and flexibility to accommodate changing patient needs.
  • Maintain accurate and detailed records of patient progress, treatment plans, and outcomes.

Requirements:

  • Bachelor's or Master's degree in Physical Therapy (DPT or PT)
