Occupational Therapist (OT)
Provide home health visits to adult and geriatric patients in designated areas, performing thorough evaluations of functional, cognitive, psychological, and motor skills.
Develop and continually revise individualized intervention plans to promote clients' health, safety, and independence.
Assess home environments for hazards and recommend equipment or environmental adaptations.
Instruct patients, families, and caregivers in rehabilitative activities, documenting all observations and interventions accurately using electronic medical records.
Qualifications & Skills- Current Vermont Occupational Therapist license
- Minimum of one year of recent OT experience
- Strong interpersonal, organizational, and problem-solving skills
- Ability to work independently and manage time effectively
- Solid computer skills; EMR experience preferred
