Overview

Occupational Therapist – General | Berlin, Vermont, United States | Start date 10/12/2026 | Duration 16 weeks | Weekly pay $2,355–$2,393 per week | Guaranteed Hours 0 | Shift 5x8 Days | Setting:
Long-term care (LTC) nursing home. Rates are tiered by mileage (direct line) for Travel assignments: 101 miles+ qualifies as Travel, 51–100 miles is Radius, 50 miles or less is Local. This assignment requires Vermont OT licensure (may be pending) and BLS certification.

Responsibilities
  • Perform comprehensive OT evaluations for residents in a long-term care setting.
  • Develop and implement individualized treatment plans addressing activities of daily living, mobility, cognition, and safety.
  • Monitor progress, document outcomes, and adjust plans in collaboration with the interdisciplinary team.
  • Provide patient and family education on therapeutic exercises, adaptive equipment, and safety strategies.
  • Maintain accurate records in the facility’s EMR and participate in care planning meetings with nursing and medical staff.
  • Attend bi-weekly orientations on Mondays and comply with site onboarding and policies as required.
Requirements
  • Vermont OT license required (may be pending)
  • BLS certification required
  • Prior travel experience required
  • Ability to work in a long-term care setting
  • 40 hours per week availability; schedule may include Monday–Sunday with bi-weekly orientations on Mondays
