Gifford Health Care is seeking a Licensed Mental Health Clinician.
The Mental Health Clinician will support patients with complex behavioral health, substance use, and social-emotional needs within Gifford's Addiction Med practice. This position provides comprehensive psychosocial and cognitive/behavioral assessments, brief intervention, care coordination, and referral to appropriate treatment and community-based services. The clinician will work collaboratively with patients to develop individualized action plans, support behavior change and recovery goals, provide counseling to reduce harmful or addictive behaviors, and help prevent relapse.
As part of Gifford's Blueprint for Health Spoke Program, this role partners closely with primary care teams, community providers, and social service organizations to deliver comprehensive care management, transitional care support, care coordination, individual and family support, and referrals for housing, transportation, employment, parenting resources, financial supports, and other community supports. Candidates must be a master's prepared or licensed Social Worker, Mental Health Counselor, Marriage and Family Therapist, or Psychologist.
