• Administer medications and treatments per physician orders and document administration accurately
• Monitor resident health status, assess for changes, and report to the care team
• Provide and document basic nursing care and support for activities of daily living (ADLs)
• Perform routine assessments and record vital signs, reporting significant findings promptly
• Collaborate with interdisciplinary team to update care plans and ensure continuity of care
• Maintain safety, infection prevention, and adherence to regulatory standards for resident care

• Active LPN license (unrestricted)
• Graduate of an approved practical nursing program
• 1+ year of experience in long‑term care or skilled nursing settings preferred
  
  Ability to work in a long‑term care setting and be flexible for day and/or evening shifts
• Strong communication and teamwork skills
