Location: Bradford
The Licensed Practical Nurse (LPN) provides direct patient care in a substance use disorder (SUD) treatment setting, including detoxification and/or residential services. The LPN functions under the direction of a Registered Nurse (RN), Advanced Practice Registered Nurse (APRN), or physician and is responsible for implementing nursing care plans, monitoring patient status, and supporting recovery-oriented accordance with Vermont law, the LPN practices within a directed scope, contributing to patient assessment, implementing care, and participating in evaluation under supervision.
Key Responsibilities Clinical Care & Monitoring Provide direct nursing care to patients undergoing detoxification or substance use treatment
Monitor and document vital signs, withdrawal symptoms, and overall patient condition
Administer medications (including detox protocols) as ordered by a licensed provider
Observe and report changes in patient condition to supervising RN or provider
Assist with medical procedures such as wound care, specimen collection, and basic treatments
Care Coordination Contribute to the assessment process by gathering patient data (history, symptoms, behaviors)
Participate in the development and modification of individualized care plans under clinical supervision
Implement nursing interventions and support continuity of care across shift transitions
Collaborate with interdisciplinary team members (counselors, case managers, physicians)
Addiction Services Support Monitor and document withdrawal using standardized tools (e.g., CIWA, COWS) (if applicable to program)
Support medication-assisted treatment (MAT) protocols as directed
Provide patient education on recovery, relapse prevention, and medication adherence
Promote a therapeutic and trauma-informed environment
Safety, Compliance & Documentation Maintain accurate, timely, and compliant medical records
Adhere to Vermont Nurse Practice Act and facility policies
Ensure patient safety and infection control standards are maintained
Participate in quality improvement and incident reporting activities
Delegation & Team Support Delegate appropriate tasks to nursing assistants or support staff, within scope and under supervision
Support daily operations of the unit, including admissions, discharges, and transfers
Minimum Qualifications Current, active Licensed Practical Nurse (LPN) license in the State of Vermont Graduate of an approved practical nursing program (certificate or associate-level)
Basic Life Support (BLS) certification (or ability to obtain upon hire)
Preferred Qualifications Experience in substance use disorder treatment, behavioral health, or detox settings
Familiarity with MAT (e.g., buprenorphine, methadone, naltrexone)
Experience with electronic health records (EHR)
Knowledge of trauma-informed care and recovery-oriented practices
