- Assess and manage acute changes, chronic conditions, and post-hospital transitions
- Lead daily rounding, including review and triage of overnight events with nursing staff
- Support safe and efficient discharges back to primary care providers
- Collaborate closely with physicians, nurses, rehab therapists, and care managers
- 0.6 FTE (30 hours/week)
- Three 10-hour shifts per week (typically 7:00am - 6:00pm)
- Rotating weekend phone call (7:00 AM - 7:00 PM, from home); paid call coverage
- A collaborative, interdisciplinary care model across a 150+ bed facility
- Care for both long-term residents (90-100) and short-term rehab patients (50)
- A highly team-oriented environment with strong nursing and leadership support
- Meaningful continuity of care
- NP or PA with (or eligible for) Vermont licensure
- BLS certification required
- 2+ years of experience in primary care, hospital medicine, urgent care, or post-acute care
- Comfort managing medically complex adult or geriatric populations
- Ability to work independently while collaborating closely with a multidisciplinary team
