WhoWe're Seeking
We're seeking a Licensed Practical Nurse (LPN) to join our Home Health teams. Your role involves monitoring and reporting on patients' medical and emotional well-being. Additionally, you'll communicate with the overseeing RN to coordinate support from the home health team. This position focuses on delivering care to individuals in their homes.
Your Responsibilities- Determine patient care needs and develop goal-oriented care plans in consultation management.
- Ensure accurate and timely documentation of patient information, adhering to HIPAA regulations.
- Work autonomously under RN direction and utilize supervisory support when necessary.
- Empower clients to explore care options and alternative plans to achieve desired outcomes.
- Participate in on-call rotation and respond to calls promptly.
- 28 days combined time off in your first year for a healthy work-life balance.
